Each form included in the athletic package document MUST be completed as follows: 

1.    .   The first two pages are to be completed by the student/athlete’s parent or guardian.
2.    ..  The last 3 pages of the package are to be completed by the medical office/physician conducting the physical exam.
3.    .   Submit all completed forms to the Athletic Director’s office.
